Mr Middleton runs an extensive surgical practice in the management of disorders of the abdomen, colon, rectum and anus. His major clinical interests include the modern management of haemorrhoids. He is Medical Director of the British Haemorrhoid Centre and Reading HALO-centre, which specialises in safe, effective and pain-limited haemorrhoid surgery. His other interests include minimally invasive (keyhole) surgery for colorectal diseases such as bowel cancer, inflammatory bowel disease (Crohn's disease and Ulcerative Colitis), appendicitis and gall bladder disorders. He treats all other anorectal conditions, including bleeding, anal fissure, fistula and abscesses. He conducts extensive bowel investigations by means of colonoscopy and endoscopy.

Mr Middleton is an experienced colorectal and general surgeon, with a large practice built up since he was appointed as consultant in 2001. His higher surgical training was carried out in the South-West Thames area and he completed his Certificate of Completion of Specialist Training (CCST) in 2001, with sub specialty training in colorectal surgery. That year he was also awarded a Master of Surgery degree (MS) following a two year period of research at St Mark's Hospital at Northwick Park in Harrow.
